This study is being conducted by Eunsaem Kim, a Prevention Science doctoral student in Washington State University. The Washington State University Institutional Review Board reviewed this study. This information is for his Master’s Project and your participation is greatly appreciated.

The purpose of this survey is to identify factors related to physical activity for older adults with Alzheimer’s disease (AD) and other dementias. In particular, he would like to examine the role of caregivers in AD patients’ physical activity participation. The information you provide will be used to help promote well-being, and increase the quality of life for older adults with AD.
