Let Your voice Be Heard!
As part of the development of the World Alzheimer Report 2012, to be launched on 21 September, ADI is currently conducting a survey of people with dementia and family caregivers to find out their opinion and experience of stigma related to dementia. The intended audience is only people with dementia and family or informal care partners. Professional carers are not being asked to participate at this time.
Please complete the survey as individuals, and not as a pair. For example, if you are a person with dementia, answer each question in your own opinion (even if you have a carer helping you). As a family carer, please begin a new survey and answer the questions in your own opinion.
Responses are anonymous. All questions provide multiple choice answers. Additional comments are welcome and are optional.
